HOW IT WORKS
Instant download. Open the Word files, replace the bracketed placeholders with your agency details, and print or send them for e-signature. The PDF is a reference copy. The Excel workbook works in Microsoft Excel and Google Sheets.
You brand the files yourself: add your agency name and logo to the editable templates. Licensed for use inside one agency only. Not white label, and not for resale, sharing or redistribution.
FAQ
Q: Does this work in my state?
A: The templates are written for use in any state, with notes where state law commonly differs. The state tables show where to check your rules. Have a local employment attorney review your final versions.
Q: Can I use a non-compete with my caregivers?
A: In many states, no. The packet explains the 2026 status, including the end of the FTC rule, and gives you a non-solicitation agreement to use instead.
Q: Is this only for Medicaid agencies?
A: No. EVV and exclusion check sections apply to Medicaid and Medicare billing, and you can delete them if you are private pay only.
Q: Can I edit everything?
A: Yes. Every template is fully editable.
